As of February 2009:
1. Completed visa form (form distributed to passengers in-flight to Bolivia 
    & at any Bolivian border if arriving by land
).
    Cost of the visa:  US$ 135.00 per person
2. Passport valid for 6 months beyond completion of trip
3. Proof of yellow fever vaccination
    (required only when travelling to high risk areas – traditional tourist routes
    such as La Paz, Sucre and Santa Cruz are not at risk).

4.  Present travel itinerary (hotel and means of departure)
5. Travellers arriving in Bolivia should be prepared to demonstrate sufficient
    financial means to support their stay. Travellers will need to be in possession
    of a credit card or a sufficient amount of cash equal to approximately
    USD 50 per day per person.
NOTE: Many countries are requiring that passports be valid for at least 6 months
after the end of travel
